FOOD VENDOR FORM

Vendor fees are non-refundable. $25 Vendor Fee is due upon acceptance. 

You will receive one (1) table and two (2) chairs for your set up.

Please attach a copy of the following documents with your application:

• Copy of Food Handler’s Permit
• Health Department Food Vendor Permit
• Business License (if applicable)
• Bartending License (if applicable)

By submitting this form, you acknowledge and agree to the following:

• The $25 vendor fee is non-refundable
• You will comply with all local health and safety regulations
• You will arrive on time and stay for the full duration of the event
• You are responsible for the clean up of your vending area
• You release the event organizers from any liability related to food provided
